Senate Bill 12 went into affect on September 1, 2025.  All public schools must obtain annual parent consent before providing any health care services. Without this form, the school nurse must call parents and receive written permission to treat their child. 
Skyward now has a new tile that will allow you to give or not give consent. The whole process takes less than 5 minutes. Please log into your child's Skyward account and click on the TILE labeled: SB12 Health Services.
